This
year Westfield Crew will be utilizing online registration.You will find a Registration link at the bottom of this page.The deadline for both online registration and
paper forms (see below) has been extended to March 21.
FCPS
regulations require that five forms be completed on paper.Once completed, please put these forms in an
envelope marked "Crew Registration" and return them to the Crew Box in the Activities
Office at the high school.You will be
asked to print out forms one, two and three when you register online.The five forms required for complete registration are:
Field Trip Driver's License and Vehicle
Insurance Information - one form must be completed for EACH DRIVER who will be
driving anyone other than their own family member(s).

Participation Policy***Note that you must complete the registration process after printing these three forms. You will receive a confirmation number from Ronin when you have completed the online part of registration.
Emergency Care Information - You will find a filled-in printable copy of your child's emergency care form if you sign into your Blackboard account. Please make any necessary corrections then sign and date it. Blank forms are available on the school website or in the Activities Office.
VHSL Physical - Each athlete must have a physical (completed after May 1, 2009) on file in the Activities Office. You must use the VHSL (Virginia High School League) form which is available on the school athletics website or in the Activities Office. If you participated in a fall or winter sport or have already turned in a physical this year, please email info@westfieldcrew.org your name and what sport you participated in.

Dues
this year are $495 which includes two tickets to Basket Bingo (one of our major
fundraisers on February 6, 2010) which may be sold for $15 each or used by your
family.The dues may be paid in three
installments if desired.Payment coupons
are below.
Uniforms are additional and
are estimated to be approximately $65.If you have last year’s uniform you do not need to buy a new one. More information about uniforms will be sent in an email.
